Application for Disabled Person Pension Scheme

The eligible age for the pensioner is 18 years and above and the person with disability level has to be 80%. The amount is Rs. 300 per month and after attaining the age of 80 years, the beneficiary will get Rs. 500 per month. Dwarfs will be an eligible category for this pension.

• ID cum Residence proof of the Parent/ Guardian ( Only in case applied by Parent or Guardian )

• The first page of the Passbook

• Self Declaration

• Proof of disability like a UDID Card or Disability Certificate

• Date of Birth Proof of Person with Disability A. Aadhar Card and B. Voter Card or Voter List or Birth Certificate or Matriculation Certificate

• Proof of Identity and Proof of Address of Beneficiary
